OVERVIEW
To coordinate and act as a knowledgeable resource in assisting Naskapi children and families, allowing them access to existing federal, provincial, and local services. Such as social services and educational programs that address a child’s needs.
REQUIREMENTS
- Bachelor’s degree in a relevant field; otherwise a Certificate or Diploma in a relevant field with two (2) years’ experience; otherwise a Community-based worker or Cultural Worker;
- Knowledge of health and social services and educational supports available to Naskapi Beneficiaries;
- Effective advocacy and communication skills. Including Naskapi, Innu, English and French;
- Comfortable in public speaking and facilitating information sessions;
- Strong organizational, critical thinking, multitasking, and decision-making skills;
- Computer literate;
- Openminded to feedback from clientele;
- Committed to a healthy lifestyle and willing to undergo drug testing.
JOB DESCRIPTION
- Develop and maintain relationships with community resources, including collaboration with existing community-based organizations;
- Proactively identify and support children with unmet needs, facilitating timely access to services;
- Create and implement case management policies and procedures, ensuring effective service planning and evaluation;
- Liaise with health and social services, fostering collaborative relationships to enhance service accessibility;
- Facilitate access to professionals for assessments and follow-ups with clients to ensure continuous care for children;
- Promote cultural competency in care, treating First Nations Peoples with respect and compassion.
- Coordinate with Jordan’s Principle focal points for complex cases, ensuring seamless service transition;
- Establish networks with the CLSC, Wellness Team and other services;
- Process service requests, liaising with Health Canada for approvals and follow-ups;
- Inform sectors about the initiative, maintain records for reporting, and facilitate consultation meetings.
